Home blood pressure monitoring

We are in the process of streamlining our processes for monitoring blood pressure. As such, we are phasing out paper blood pressure forms and diaries. If you are required to monitor your blood pressure at home, you will be requested to do so by a clinician.

Your clinician will either:

  • Send you a text with a daily reminder and link to submit a reading each day for 4 or 7 days. You will need an internet enabled mobile phone to be able to do this.

OR

  • Direct you to our pharmacist, Sherry who operates a walk-in blood pressure monitoring service Monday-Thursday. This option is best suited to patients who do not have a smartphone.

If you wish to continue monitoring your blood pressure at home, you are welcome to do so. If you have concerns around your blood pressure and need clinical advice, please call the surgery to arrange an appointment.

From 1st December 2022, we will no longer be accepting any paper blood pressure diaries.
