Medication Home Delivery – COVID-19

**Since the introduction of the practice COVID-19 medication home delivery service, we have received a huge volume of requests. Please help us to help our most vulnerable patients by only requesting home delivery of your medication if you absolutely cannot leave your home (including for shopping and medical appointments), and do not have a trusted friend, neighbour, or family member who can collect your medication for you.**

If you require delivery by the practice, please use one of the below methods to submit your request in writing and an authorised group of community volunteers will be instructed by the practice.

  1. PREFERRED OPTION – Send your request (including your name, address, and date of birth) to our team via the practice website HERE.
  2. Email your request (including your name, address, and date of birth) to pm.downland@nhs.net.
  3. Send your request (including your name, address, and date of birth) to the practice by post.

If you have been receiving the medication home delivery service and no longer require it, or you have a friend, neighbour, or family member who can collect your medication for you, please notify us via the website ASAP and allow us to give your delivery slot to a vulnerable patient.
